Alot of this problem is how your arm is actually going to the desk.
I've done consulting in Ergonomics and see the most ridiculous set ups. One woman had her monitor about 100 degrees off her left side! When asked why she responded "I like to look out the window, and i dont have to see the monitor, i am a good typist" She actually used hotkeys to save and everything and would only look over when web surfing. I managed to get her to at least have the monitor in the forward arc now, hehe.
Anyway, you should try a desk made by a company called Anthrocart, http://www.anthro.com ive had mine for 8 years now, and its still the best ive seen. Browse their site and spend the 100 to 200$ (more if you want something huge like mine)your whole body will thank you.
Also, do not skimp on human interface equipment, get a ergonomic kb and mouse, we are talking 80$, your wrist is worth it.
When you get your new desk make sure that its adjusted to a comfortable height.
You will also need a chair that is comfortable.
Make sure that the top of your monitor (the actual screen) is 1 inch above your eyeline (where a straight line drawn from your pupil would end up).
Start with that, and adjust until you feel comfortable. You might have to spend 300$ or 400$ doing this but its better than the 30,000$ you would need in doctors bills if this gets out of hand.

P.S. Get the largest monitor you can afford and make sure the refresh rate is at (or well over) 75hz
P.P.S. The instructions that came with the game say to take a break every hour, i know that's as likely as the Dregnin given me Ranger tech for Zero G manufacturing, but i figured id mention it.
