I mean the title of the game, is it reference to some enchantress who died during Cataclysm or is it some enchantress produced by Titans(i.e. of the Fallen race)?

If I'm not mistaken, the Fallen Enchantress makes reference to Ceresa, the Oracle and leader of the Wraith race. The story of how she became the fallen enchantress is part of the 1st game's campaign
