The first screen snapshot, below, shows a dialog that comes up after surveying an anomaly. When you click on the button to close the dialog, you can search 'til the cows come home, but you will not find this new planet. However, if you save the game then reload it, the new planet will appear.
The second screen snapshot below shows the area around Prima Hyadum after closing the dialog. Note that there are not any habitable planets there. However, it you save the game, then reload it, a wonderful new planet appears as shown in the third screen snapshot below.

Here are the steps to recreate the issue:
- Load the above save file
- Click on the icon in the lower right-hand corner of the screen and the dialog shown in the first screen snapshot below will appear
- Click on the button to close the dialog
- Go to the star named Prima Hyadum, just up and to the left of my home world, Neuraxia Prime
- you should see that there are no habitable planets around that star as shown in the second screen snapshot below
- Hit Esc and click on the "Save Game" button, giving the save file any name that you want
- Hit Esc and click on the "Load Game" button, selecting the save file you just created in step 5 above
- After the file loads, go bact to the star named Prima Hyadum and you should now see the new habitable planet as shown in the third screen snapshot below
To fix this issue, I believe all you need to do is redraw the planet that was just made habitable (I do not believe a new planet is created, I believe a dead world is made habitable). I believe this is what happens when you use the artifact that brings a dead world to life so you should be able to do the same thing here.
